29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: 1.) On or about December 3, 2015, the employer failed to ensure that the Nissan powered industrial truck (Serial #MCP1F2A25LV) in need of repair was taken out of service until restored to safe operating condition. Employees were exposed to struck-by, and fire hazards from the clamp securing the propane tank not locking properly.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: 1.) On or about December 3, 2015, the employer failed to train employees on the elements of a written hazard communication program including the hazards of the chemicals in their work area. Employees were exposed to hazards from carbon monoxide gas from the exhaust of propane powered forklifts used in the facility.
