29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:     On or about December 3, 2015; on the south side of the jobsite, the employer allowed employee(s) to utilize a Genie forklift with a working platform/basket that was not attached to the forks of the forklift. The employees were standing in the basket while installing metal sheeting to the outside of the steel building.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    On or about December 3, 2015; on the south side of the jobsite, the employer allowed employees to utilize an extension cord that was damaged due to overcurrent conditions. Employees used the extension cord as a power source for a Dewalt screw gun.
