29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) Facility, 1701 Morrell Avenue, Connellsville, PA:  On or about December 2, 2015 a point of operation on the densifying machine was not guarded to prevent employees from having any part of their body in the danger zone during operation.  The hydraulic door where the product exits the machine was not guarded.  An employee used their foot to move debris from the area under the door while it was open.  The door closed suddenly and crushed the employee's right foot.  The employee suffered amputation of the toes on their right foot as a result of the incident.    (b) Facility, 1701 Morrell Avenue, Connellsville, PA:  On or about December 3, 2015 the point of operation on the Edwards cable cutter to prevent employees from having any part of their body in the danger zone during operation.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  Facility, 1701 Morrell Avenue, Connellsville, PA:  On or about December 3, 2015 the employer did not establish a program that consisted of energy control procedures, employee training and periodic inspections.  Employees perform servicing on the balers and other mechanical equipment and the employer does not have an energy control program to ensure employees are not exposed to the unexpected energization, start up or release of stored energy of any of the equipment.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:    (a) Facility, 1701 Morrell Avenue, Connellsville, PA:  On or about December 3, 2015 and December 23, 2015 oxygen and propane cylinders were stored together in the scrap weighing area and were not protected from vehicles or powered industrial equipment that is operated in the area.
