29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On December 3, 2015 during an inspection the employer had employees working in the main production area of the facility located 1407 2nd Ave, Sutton, NE where the employer has failed to adequately clean the facility resulting in combustible wood dust accumulations in excess of 1/32 of an inch.  Instances include but are not limited to:  i.    Wood dust build-up on the overhead rafters exceeding 1/32 of an inch.   ii.   Wood dust build-up on unused machines in the main production area approximately ¼ inch thick.  iii.  Wood dust build-up on wall beams along the north wall of the main production area ¾ inch thick.   iv.  Wood dust build-up in the dust collector room along all wall beams and overhead beams approximately one inch thick.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    On December 3, 2015 during an inspection the employer had employees operating two separate horizontal band saws to perform re-saw work whereby they cut 2x4s into 2x2s in the main production area of the facility located 1407 2nd Ave, Sutton, NE; in the days prior to the inspection the employer has had two employees changing the blade on the re-saw while only having one employees lock applied to the lockout mechanism, creating electrical and amputation hazards.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:    On December 3, 2015 during an inspection the employer had employees operating two separate horizontal band saws to perform re-saw work whereby they cut 2x4s into 2x2s in the main production area of the facility located 1407 2nd Ave, Sutton, NE; the employer has failed to adequately guard these saws in that the non-cutting area located to the right of the cutting area has left approximately 5 inches of exposed blade, creating an amputation hazard.
