The employer, LORENZO CONSTRUCTION CORP., did not submit to the agency documents demonstrating that abatement is complete for each serious violation for which the Agency indicates in the citation that such abatement documentation is required, on our about January 25th, 2017. More specifically LORENZO CONSTRUCTION CORP did not submit to the agency documents demonstrating that the operator of the all terrain forklift was certified.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.      a) Exterior of Building - Scaffold: Employees were exposed to a fall hazards of approximately 24 feet while working from a scaffold platform that was approximately 24 feet in height  and 104 feet wide. The scaffold was not properly tied off or secured as they engaged in masonry activities, on or about 11/24/15

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.      a) Exterior of Building - Scaffold: Employees without any means of fall protection, engaged in masonry activities from a welded frame scaffold platform, were exposed to a fall hazards of approximately 24 feet, on or about 11/24/15.      LORENZO CONSTRUCTION CORP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 315640284, citation number 02, item number 02 and was affirmed as a final order on 9/28/12, with respect to a workplace located at 305 Inman Ave., Colonia NJ.
