29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location: 538 E Passaic Ave., Bloomfield, NJ  a) Employees were not protected from falling while working from a pump jack scaffold that did not have a complete guardrail system and they were not wearing a personal fall arrest system.  The employees were exposed to a potential fall of up to approximately 20 ft. to the floor below; on or about 12/1/15.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Location: 538 E. Passaic St., Bloomfield, NJ.  a)The ladder's side rails used to access the pump jack scaffold's platform extended approximately 1 ft. above the landing surface.  Employees were exposed to fall hazards of up to 20 ft. to the ground below; on or about 12/1/15.
