29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems.  (a) Worksite was located at 299 Cherry Hill Road, Parsippany, NJ 07054.  An employee was observed hoisting material up the side of the building leaning over the edge of the roof with his foot up on the parapet wall with no fall protection.  Violation was observed on or about 11/30/2015.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Worksite was located at 299 Cherry Hill Road, Parsippany, NJ 07054.  Employees were observed working within the controlled access zone with no dedicated safety monitor present and not wearing fall protection.  Violation was observed on or about 11/30/2015.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) Worksite was located at 299 Cherry Hill Road, Parsippany, NJ 07054.  Employees were using a robe grab as an improvised anchorage point.  Violation occurred on or about 11/30/2015.

Employees did not use the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems that were used nor did they know the limitations of the mechanical pieces within the systems used.  Violation was observed on or about 11/30/2015.
