29 CFR 1926.453(b)(2)(v):  A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At the jobsite located at 1719 N Rock Road, Derby Kansas:  Employees engaged in facade installation activities using an aerial lift were exposed to fall hazards in that employees wearing safety harnesses were not attached to the aerial lift.  The EFI Panels, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1018687, citation number 1, item number 1 and was affirmed as a final order on July 1, 2015, with respect to a workplace located at 2701 NE Bob Bullock, Laredo, TX 78043.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage, sharp corners and projections, or pinch points.   Employees engaged in exterior paneling activities were exposed to electrical shock hazards in that the extension cord for the circular saw was not protected from vehicle traffic which caused a pinch point.
