29 CFR 1910.1200(h)(1): Employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    (a) DeAndrea Coring and Sawing Inc, at US Highway 36, Louisville, CO: On and preceding 12/3/15, the employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment.  An employee used a Husqvarna model FS6000L walk behind saw to cut concrete panels during the US Highway 36 construction project.  The employee had not been provided training on hazardous chemicals in his work area, including but not limited to the following:    (1)  Concrete dust (containing respirable silica);  (2)  Harsco Black Beauty Abrasives and Roofing Products (containing silicon dioxide, aluminum oxide, calcium oxide);  (3)  Dow Corning Corporation Dow Corning 888 Silicone Joint Sealant (containing dimethyl, methylethyl-N-hydroxyethamine siloxane); and  (4)  Dow Corning Corporation Dow Corning 890-SL Silicone Joint Sealant (containing methylvinyl bis(N-ethylacetamido) silane).

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace:  (a) DeAndrea Coring and Sawing Inc, at US Highway 36, Louisville, CO: On and preceding 12/3/15, the employer did not identify and evaluate the respiratory hazard in the workplace.  An employee used a Husqvarna model FS6000L walk behind saw to cut concrete panels during the US Highway 36 construction project.  Concrete contains crystalline quartz.  The sawing of concrete potentially released respirable silica.  Employee exposure to respirable silica had not been determined.  This condition potentially exposed the employee to a respiratory hazard.
