DISTRIBUTION CENTER/LOADING DOCK:  The employer did not ensure through providing appropriate trainings and instructions that all drivers were keeping a clear view of the path of travel, slowing down, sounding horn at cross aisles and utilizing other safe means at all times to prevent from colliding each other.    On or about November 22, 2015, two (2) operators, operating a motorized pallet jack and a forklift truck collided each, and one of operators sustained serious injuries from the incident.
