29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. DISTRIBUTION CENTER: The floor surfaces in aisleways 6, 7, and part of 8, toward Zone G and in/around trough/grates were very wet and had an accumulation of water on the floor surfaces.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. DISTRIBUTION CENTER/LOADING DOCK: The employer did not ensure through providing appropriate trainings and instructions that all drivers were keeping a clear view of the path of travel, slowing down, sounding horn at cross aisles and utilizing other safe means at all times to prevent from colliding each other. On or about November 22, 2015, two (2) operators, operating a motorized pallet jack and a forklift truck collided each, and one of operators sustained serious injuries from the incident.
