Employees were working at heights greater than 6 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 13916 Granada Rd., Leawood, Kansas; employees using the two portable metal extension ladders to access the roof were exposed to fall hazards of up to 20 feet from the ladder to the ground on the east side and approximately 11 feet from the ladder to the ground on the north side of the house in that the side rails of the ladders did not extend at least 3 feet above the landing surface of the roof. The side rails were extended approximately 18 inches above the landing surface of the roof.
