29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the jobsite located at 13916 Granada Ave., Leawood, Kansas; employees engaged in residential roofing activities from the 6:12 and 14:12 pitched roof were exposed to fall hazards of up to approximately 20 feet from the eave to the ground below in that fall protection was not provided.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsitel located at 13916 Granada Rd., Leawood, Kansas: employees engaged in residential roofing activities had not been trained to recognize and minimize fall hazards.  Employees were working at heights greater than 6 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 13916 Granada Rd., Leawood, Kansas; employees using the two portable metal extension ladders to access the roof were exposed to fall hazards of up to 20 feet from the ladder to the ground on the east side and approximately 11 feet from the ladder to the ground on the north side of the house in that the side rails of the ladders did not extend at least 3 feet above the landing surface of the roof.  The side rails were extended approximately 18 inches above the landing surface of the roof.
