29 CFR 1926.501(b)(9)(i): Each employee performing overhand bricklaying and related work 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a controlled access zone:   On or about December 3, 2015, at the front of the building, an employee engaged in masonry work was not protected from falling by guardrail systems with toe-boards, safety net systems, personal fall arrest systems, or a controlled access zone when standing at the top of a block wall and was exposed to falls of greater than 6 feet to a lower level.

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):   On or about December 3, 2015, at the building site, the employer did not maintain at the workplace, a safety data sheet for masonry mortar.  Employees mixed masonry mortar which exposed them to hazardous chemicals including, but not limited to crystalline silica, Portland Cement, and lime in masonry mortar.
