29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about December 2, 2015, during the installation of residential siding material at the job site located at 1071 Laurel Green Dr. NE, N. Canton, Ohio 44720; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards of heights approximately 12.6 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about December 2, 2015, during the installation of siding material on a roof with a pitch of 8/12 at a job site located at 1071 Laurel Green Dr. NE, N. Canton, Ohio 44720; the employer did not ensure an employee working on a roof of a single family home was protected from a fall of approximately 12.6 feet by utilizing a fall protection system.
