29 CFR 1910.261(l)(7)(i): On single-knife machines a guard shall be provided at a point of contact to the knife. a) On November 26, 2015 machine guarding for Winder #58 machine was not adequate, in that the slitters blades exposed employees to their rotating parts exposing operators to cuts and/or amputation hazards during the tail up procedures on the machine.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a) Flexible cord attached to the electrical motor drive for Winder #58 was not equipped with a strain relief device to prevent movement or damage to conductors.
