29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) 6286 Baltusrol Trace, Fairburn GA: Employees installing siding from a pump jack scaffold were exposed to lower level fall hazards of up to approximately 23 1/2 -feet. There was no fall protection in use.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 6286 Baltusrol Trace, Fairburn GA: An employee installing siding and cornice from a ladder set up onto a roof surface was exposed to a lower level fall hazard of approximately 11 1/2 -feet. There was no fall protection in use.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a) 6286 Baltusrol Trace, Fairburn GA: Employees who were installing siding, were exposed to a lower level fall of up to 23 1/2-feet and struck-by hazards, while working from and around a pump jack scaffold that did not have the poles secured at the bottom or middle.
