29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed. On or about December 2, 2015, one employee was fatally injured and a second employee was hospitalized when the towers supporting a cable coal bridge prematurely collapsed during their demolition. An engineering survey that addressed the demolition of the bridge's towers, including the possibility of unplanned collapse, had not been made exposing employees to struck-by and/or crushed-by hazards.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines shall be shut off, capped, or otherwise controlled, outside the building line before demolition work is started. In each case, any utility company which is involved shall be notified in advance. a) On or about December 2, 2015, one employee was fatally injured and a second employee was hospitalized when the towers supporting a cable coal bridge prematurely collapsed during their demolition. A pole mounted transformer and electrical power line located within the park side tower's fall zone had not been de-energized as required.

No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. a) On or about December 2, 2015 one employee was fatally injured and a second employee was hospitalized when the towers supporting a cable coal bridge prematurely collapsed during their demolition. A competent person was not inspecting during the pre-burning operation of the park side tower's North leg exposing employees to struck-by and crushed-by hazards.
