29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. On or about November 30, 2015, the employer did not ensure that swab machines were equipped with required guarding when employees used special hand-tools to remove cotton swabs which had become jammed in those machines.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 30, 2015, the employer did not ensure employees performed the procedures for the application of energy control by; preparing the machine for shutdown [per the 1910.147(d)(1) requirements], turning off the machine [(d)(2)], isolating the machine of all energy sources [d)(3)], affixing lockout devices to hold the machine in the off position [(d)(4)(i)], and verifying that isolation and de-energinzation of the machine had been accomplished [(d)(6)], during the unjamming of swabs from the No. 4-7 Swab Machine. Energy sources include, but not limited to, electrical and pneumatic power.
