Employees were working at heights greater than 6 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At the jobsite located at 16524 Bluejacket St., Lot #39,  Olathe, KS; employees using the aluminum extension ladder to access the roof to perform roofing work were exposed to fall hazards of up to approximately 25 from the ladder to the ground below in that the side rails of the ladder did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 30 inches.
