29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)    Front of house, adjacent to double car garage doors:     Employees worked from the improperly constructed approximate 13 foot 5 inch high scaffold platform while attaching the wire mesh to the exterior wall, on or about 12/07/2015.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    a)    Front of house, adjacent to double car garage doors:     Two approximate 15 foot long 2 x 4 planks used for the working level approximate 13 foot 5 inch high scaffold platform extended approximately 3 1/2 feet over the support while employees attached from the wire mesh to the exterior wall, on or about 12/07/2015.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:      a)    Front of house, adjacent to double car garage doors:     Employee climbed down end frame from the approximate 13 foot 5 inch scaffold platform, on or about 12/07/2015;    b)    Front of house, adjacent to double car garage doors:       Employee climbed down end frame and cross braces from the approximate 13 foot 5 inch scaffold platform, on or about 12/07/2015.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a)    Front of house, adjacent to double car garage doors:     Employees were working from the approximate 13 foot 5 inch improperly setup fabricated frame scaffold stapling wire mesh to the exterior wall, unsafe conditions included no fall protection, no proper means of accessing upper levels, platform not full planked between the uprights, and planks used for the platform extended more than 18" past the supports, on or about 12/07/2015.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
