29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): (a) U.S. Postal Service at 1960 South Industrial Road, Salt Lake City, UT, 84199: On or before December 2, 2015, the employer did not ensure maintenance employees were safely positioned away from running conveyor belts, bearings, and rollers while troubleshooting equipment. On November 23, 2015 a maintenance employee troubleshooting the conveyor take-up section on the LMS#4 placed a screwdriver on a bearing while the equipment was operating. The employees hand came into contact with a moving roller and sustained compound fractures of the arm after being pulled into the machine. This condition exposed employees to injuries caused by moving parts.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees, and to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) U.S. Postal Service at 1960 South Industrial Road, Salt Lake City, UT, 84199: On or before December 2, 2015, the employer did not ensure that maintenance employees were adequately trained to ensure they acquired the knowledge and skills necessary for the safe application, usage and removal of energy control devices on equipment, such as but not limited to LMS machines. This condition exposed employees to amputation and other injuries caused by moving parts.
