29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Heartland Specialty Converting, Inc. on or about December 02, 2015, at the address of 1025 Findlay Rd. Lima, OH: The employer failed to ensure the press rollers contained on the DC-2 Die Cutter Machine (model RP-70) were adequately guarded. An operating employee was exposed to amputations hazards at the incoming rollers located at the input end of the machine. When sending the die and material though the machine, the operators hand was within the proximity of the machines unguarded processing rollers.
