29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 100-200 South Front Street, Philadelphia, PA - The employer did not protect employees from falling through holes of the staircase more than six (6) feet above lower levels by personal fall arrest systems, covers or guardrails during demolition and construction of residences.  Employees were exposed to falls of approximately ten (10) feet. Observed on or about November 27, 2015.

Front Street, Philadelphia, PA - Employees were exposed to fall hazards in that the employer did not equip a stairway having approximately seven risers with at least one handrail.  Condition noted on or about November 27, 2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 100-200 (0130 S. Front Street) Front Street, Philadelphia, PA 19106:  The employer did not ensure that each employee performing framing work was protected from falling.  Employees were exposed to falls of approximately 24ft while standing on the top edge of an interior wall on the third level.  Observed on or about November 27, 2015.  Oliveira Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1064763, citation number 1, item number 1 and was affirmed as a final order on 7/17/2015 with respect to a workplace located at 700 North Front Street, Philadelphia, PA.
