29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed and the scaffold's structural integrity was not maintained by the user:  a) 1075 Easton Avenue, Somerset, NJ: Employee was allowed to perform stucco operations from a job-made scaffold that was required the use of duct tape to hold the planking together, effecting the scaffold's structural integrity.  Condition observed on or about December 7, 2015.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  A) 1075 Easton Avenue, Somerset, NJ:  Employee was exposed to fall hazard of approximately 6'5" from a mobile scaffold due to the use of a wood 2x6 spread across the midrails utilized to increase the height on the platform.  Condition observed on or about December 7, 2015.
