29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Roof, residential home located at 725 Milton Oaks Drive, Alpharetta, GA 30022: Employees operating Hitachi (Model - NV45AB2) Coil Roofing Nail Guns were not wearing protective eyewear. Employees operating these tools at 120 PSI were exposed to struck-by hazards which included metal collated debris and fasteners as safety glasses and  or goggles were not provided.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, residential home located at 725 Milton Oaks Drive, Alpharetta, GA 30022: Employees were exposed to fall hazards while working on a 8/12 pitched roof at heights of approximately 17-feet, 4-inches to 22-feet, 1-inch with inadequate or no fall protection systems in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) South side, residential home located at 725 Milton Oaks Drive, Alpharetta, GA 30022: The ladder provided by the employer was not secured to the structure or extended at least 3-feet past the eave to allow employees safe access to the lower levels. The ladder was extended one rung past the eave which was approximately 16-inches which exposed employees to falls of approximately 21-feet, 8-inches to the lower level.
