29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite, corner of Fisk Street and St. Johns Way, Pittsburgh, PA: On or about 12/03/2015 employees working on the top of the pre-cast concrete wall section of the structure (facing butler street) were not protected from falling approximately 12 feet to the ground below. The employees were on the pre cast wall connecting the structures using a drill and were not protected from falling by guardrail systems, safety nets or personal fall arrest systems.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Jobsite, corner of Fist Street and St. Johns Way, Pittsburgh, PA: On or about 12/03/2015 employees working on the top of the pre-cast concrete wall section of the structure (facing Butler Street) were not protected from falling objects from overhead. Employees were working in the vicinity of an overhead load and on the ground near walls with employees working overhead with no head protection.
