29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Near the hydraulic press: On or about December 03, 2015, employees were exposed to an electrical hazard, from a 220 Vac receptacle pendant, which was installed via a flexible cord, while the outer jacket of the cord was pulled away for the receptacle exposing the cords internal wiring and allowing pull to be directly transmitted to the terminal screws. b) Behind the hydraulic press: On or about December 03, 2015, employees were exposed to an electrical hazard, from a 220 Vac plug of the Acme Spot Welder, which was being powered via a flexible cord, while the outer jacket of the cord was pulled away for the plug exposing the cords internal wiring and allowing pull to be directly transmitted to the terminal screws. c) In the maintenance shop: On or about December 03, 2015, employees were exposed to an electrical hazard, from a 220 Vac receptacle used to power the Miller Arc Welder, Model 250AC/DC-HF. The receptacle's outlet box received power a flexible conduit, which had pulled away from the outlet box exposing the internal wiring and allowing pull to be directly transmitted to the terminal screws.
