29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations:    (a) Industrial Propane and Equipment, Incorporated - The employer did not develop and implement a safe means to control the potential release of propane pocketed within the desiccant nor the generation of static electric charges during the cleaning and changing of the propane desiccant, on or about November 23, 2015.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
