29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about November 25, 2015, the employer failed to ensure that the barrier guard for the Nor Service Inc. round pipe roller machine (Serial # N0R-R5-08) was effective in preventing employees from entering the point of operation.
