29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:   (a)  Building 2767, Wash Bay:  On or about December 2, 2015 and at times prior thereto, the outer casing of the 240 volt attachment plug for the Landa SEA 4-2000 pressure washer was cracked and repaired with black vinyl tape.  This condition exposed employees to electrical shock and burn hazards from contact with the damaged equipment.   (b)  building 2767, Cell 4:  On or about December 2, 2015 and at times prior thereto, the outer jacket of a red, 25 foot, 120 volt extension cord was cut.  This condition exposed employees to electrical shock and burn hazards from contact with the damaged equipment.   (c)  building 2767, EW02 Station:  On or about December 2, 2015 and at times prior thereto, the outer jacket of a Snap On 120 volt extension cord assembly was cut.  This condition exposed employees to electrical shock and burn hazards from contact with the damaged equipment.
