29 CFR 1910.23(d)(1)(i): Flight(s) of stairs with four or more risers, less than 44 inches wide and having both sides enclosed, were not equipped with at least one standard handrail.  Building 2350, stairwell leading to storage area above alarm shop; on or about 02 December 2015 and times prior thereto; a stairway measuring 42 inches in width with ten vertical risers was not equipped with a standard handrail on either side. This condition exposed employees to a fall hazard.

Building 2350, entryway; on or about 02 December 2015 and times prior thereto; a stairway measuring 82.5 inches in width with seven vertical risers was not equipped with a standard handrail on either side. This condition exposed employees to a fall hazard.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Buildings 2350, structures shop; on or about 02 December 2015 and times prior; the employer did not ensure that emergency exits lights were in proper working order. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   Building 2350, machine shop; on or about 02 December 2015 and times prior thereto; a Sound Bend Heavy Duty Gear Head Lathe, Model SB1050, Serial Number SB1050-0001; had the chuck guard removed. This condition exposed employees to a caught in hazard.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  Building 2350, Structures Shop, electrical room; on or about 02 December 2015 and times prior thereto; two instances were noted where electrical panels had items stored in front of the panel impeding immediate access.        a. A Kettler foldable ping pong table was stored in front of the main panel board.        b. Wooden planking was stored in front of the panel board adjacent to the dust collection blower fans.  This condition exposed employees to delays in de-energizing electrical equipment, electrical shock and electrical burn hazard.  The Air Force was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.303(g)(1)(ii), which was contained in OSHA inspection number 111281, citation number 1, item number 3 and was affirmed as a final order on 06 March 2012, with respect to a workplace located at Eielson Air Force Base, 354th Civil Engineering Squadron, Structures Electrical room.
