29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  Power Plant, Corrosion Inhibitor Room; On or about 38 December 2015 and times prior thereto; the employer did not ensure that the work areas were clean and orderly.  The room had numerous items loosely scattered on the floor impeding the walking and working surface. This condition exposed employees to a tripping hazard.

Power Plant, Corrosion Inhibitor room; On or about 3 December 2015 and times prior thereto; emergency lighting was not available in the Corrosion Inhibitor Room.  This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  Power Plant, Pipe fitters/Mechanic area;  On or about 3 December 2015 and times prior thereto, an air nozzle used for cleaning purposes exceeded 30 p.s.i. The air nozzle measured approximately 100 psi.  The air nozzle was not equipped with a pressure deflector to reduce the pressure of air flow.  This exposed employees to foreign objects being embedded to the body and eyes.
