29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Throughout the welding shop area; on or about 03 December 2015 and times prior, the employer did not ensure that the work areas were clean and orderly. Welding area had numerous items loosely scattered on the floor impeding the walking and working surface. This condition exposed employees to a tripping hazard.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded. Welding room work bench area; on or about 03 December 2015 and times prior thereto; a floor hole measuring approximately 18 inches by 14 inches, with a depth of approximately 10 inches, was not guarded. This condition exposed employees to a tripping hazard.

29 CFR 1910.254(d)(8): Welding cables with splices within 10 feet (3 m) of the holder were used. Welding shop; on or about 03 December 2015 and times prior; two instances were noted were welding cables had damage within 10 feet of the holder: a. Miller Gold Star 302 stick welder, serial number KH445178 had damaged on the stinger cable at 4 inches from the holder. b. Lincoln Model TIG 300/300 AC/DC welder, serial number 31140 had damage on the stinger cable at 96 inches from the holder. This condition exposed employees to electrical shock and burn hazards.
