29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) At 2700 Henrietta Street in St. Louis, MO 63104 On or about 12/08/2015, the employer failed to provide a training program for each employee exposed to fall hazards, approximately 55 feet or more above a lower concrete surface, to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize those hazards. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
