29 CFR 1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times:         a) On or about November 20, 2015, at 3051 Kumho Parkway, Macon, Georgia: employees working in the curing building were exposed to struck-by hazards from exploding compressed gas cylinders when several LP gas tanks were left unsecured in the work area in which employees operating motorized equipment including forklifts, scissor lifts, and articulating boom lifts.         b) On or about December 15, 2015, at 3051 Kumho Parkway, Macon, Georgia: employees working at the outside NE corner of the mixing building were exposed to struck-by hazards from exploding compressed gas cylinders when five Argon compressed gas cylinders were stored on their side at the outside entrance in which employees operating motorized vehicles including forklifts, scissor lifts, and articulating boom lifts.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about December 15, 2015, at 3051 Kumho Parkway, Macon, Georgia:        a) An employee performing roofing work on the shipping warehouse canopy was exposed to a fall hazard of approximately 32-feet to ground level when the employee operated an articulating boom lift without attaching his lanyard to the basket.         b) An employee performing sheet metal installation work on the main office building canopy was exposed to a fall hazard of approximately 20-feet to ground level when he operated an articulating boom lift without attaching his lanyard to the basket.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about December 15, 2015, at 3051 Kumho Parkway, Macon, Georgia:        a) Five employees performing roofing work on the shipping warehouse canopy were exposed to a fall hazard of approximately 120-foot fall to ground level when they worked at the unprotected edges of the building without wearing harnesses or while wearing harnesses without attaching their lanyards to an anchorage.         b) An employee performing roofing work on the shipping warehouse canopy was exposed to a fall hazard of approximately 32-feet when he worked at the unprotected edge of the canopy roof without tying off his lanyard to an anchorage.         c) Two employees performing sheet metal installation work were exposed to a fall hazard of approximately 20-feet when they worked at the unprotected edge of the Main office building canopy without tying off their lanyards to an anchorage.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    On or about November 19, 2015, to January 19, 2016, at 3051 Kumho Parkway, Macon, Georgia:        a) An employee performing electrical wiring installation work  was injured ultimately resulting in his death when he fell from Catwalk 2 in the curing building when he stepped though an unguarded floor opening while not having his lanyard tied off to an anchorage point.         b) Employees working for GMC Blue Power Construction were exposed to fall hazards of approximately 7-10-feet while walking, working, and spotting boom lifts in the proximity of three approximately 100-foot long x 50-foot wide curing pits when the pits were not guarded, covered, and the employees did not use fall protection.         c) Employees operating powered industrial trucks and working in the curing building for KG USA, Inc. were exposed to fall hazards of approximately 7-10-feet while walking, working, and spotting boom lifts in the proximity of three approximately 100-foot long x 50-foot wide curing pits when the pits were not guarded, covered, and the employees did not use fall protection.         d) Employees working for Samsung Steel in the curing building were exposed to fall hazards of approximately 7-10-feet while walking, working, and spotting boom lifts in the proximity of three approximately 100-foot long x 50-foot wide curing pits when the pits were not guarded, covered, and the employees did not use fall protection.         e) An employee working in Building A, at the Mixer Operator Level for Sung Won was exposed to a fall hazard of approximately 13-feet when the employee did not attach his lanyard to an anchorage point while painting around and uncovered floor hole measuring approximately 10-feet by 10-feet.         f) Temporary employees operating floor cleaning machines for Premier Staffing, Inc. in the curing building and in Building A, were exposed to fall hazards of approximately 7-10-feet in proximity to the curing pits which were approximately 100-feet long and 50-feet wide and various depths and dimensions in Building A, when the floor openings were not covered.         g) Employees inspecting machines from in the curing building working for Kumho Tire Asiana were exposed to fall hazards of approximately 77-inches deep when they worked in the proximity of floor openings of approximately 2-feet wide and 10-feet long when they did not attach their lanyards to an anchorage point.         h) Two employees installing skylights on the Building H canopy roof were exposed to a fall hazard of approximately 20-feet when they did not use fall protection while working around the unguarded floor opening.
