29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about November 19, 2015, on Catwalk 2 in the Curing Building at 3051 Kumho Parkway, Macon, Georgia: Employees supervising and installing electrical wiring were exposed to a fall hazard of approximately 16-feet through an unguarded floor opening where the employer did not ensure that employees were tied off. This resulted in an employee falling through an unguarded floor opening injuring the employee and later resulting in his death.
