29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On or about December 8, 2015, at the above addressed site, an employee was exposed to a fall hazard of approximately 25 feet when installing Christmas wreaths onto existing light poles from the bucket of a truck mounted aerial lift while not wearing a safety belt and/or a harness with a lanyard attached to the anchor provided.
