29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):  (a) Date of Violation: On or about 12-17-2015 and times prior thereto located at Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, in the Carpentry Shop, employees were exposed to falls from a 9 feet, 5 inch wide open-sided, unguarded mezzanine work platform at a height of 15 feet, 4 inches above the ground level below.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.213(p)(4): Belt sanding machinery was not provided with a guard at each nip point where the sanding belt ran onto a pulley, to prevent the operator's hands or fingers from coming into contact with nip points:  (a)  Date of Violation: On or about 12-17-2015 and times prior thereto located at Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, in the Carpentry Shop, employees were exposed to unguarded rotating parts at the sanding belt pulley which also had a smaller rotating sanding drum attachment mounted on top of the belt pulley which placed the point of operation directly above the unguarded rotating pulley and drum.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

The work rests was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   (a)   Date of Violation: On or about 12-17-2015 and times prior thereto located at Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, in the Mechanic Shop building 8, employees were exposed to a floor mounted pedestal grinder that contained a work rest with adjustment exceeding one-eighth inch. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  (a)   Date of Violation: On or about 12-17-2015 and times prior thereto located at Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, in the Carpentry Shop paint mixing room, employees were exposed to electrical hazards from an extension cord constructed with wiring and a metal wall-mounted style outlet box and the strain relief fitting was not connected to the outlet box therefore putting strain directly on the terminal screws internal of the box. Cord was used to supply power to a paint shaking machine.  Abatement certification required within 10 days after abatement date. The United States Department of Veterans Affairs was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1074778, citation number 2, item number 2 and was affirmed as a final order on 08-08-15, with respect to a workplace located at U.S. Department of Veterans Affairs Fresno, CA 93703.  Abatement certification required within 10 days after abatement date. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a)  Date of Violation: On or about 12-17-2015 and times prior thereto located at Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, employees were exposed to electrical hazards from unused openings located in electrical enclosures.  Instance #1:  Warehouse storage room.  A blanked off junction box contained an unused opening whereas a "knockout" was missing exposing the internal electrical parts of the junction box.  Instance #2:  Mechanics Shop, An EATON Cutler-Hammer electrical disconnect panel contained an unused opening whereas a "knockout" was missing exposing the internal electrical parts of the panel.  This panel displayed warnings of arc flash hazard.  Instance #3:  Carpentry Shop, A power receptacle located underneath the Delta Belt Sander contained an unused opening whereas a "knockout" was missing exposing the internal electrical parts of the receptacle box.
