29 CFR 1926.761(b): The employer did not institute a training program and ensure employee participation in the program: On Or about 12/8/2015 at 3602 Lawrenceville-Suwanee Road, Suwanee, GA; two employees performing wood cutting operations of a 2 X 6 while standing on an awning were not provided training in order to recognize the hazards of falling during steel erection activities.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about 12/8/2015 at 3602 Lawrenceville-Suwanee Road, Suwanee, GA; Two (2) workers performing wood cutting operations of a 2 X 6 were observed to access and egress the awning where the Werner 24 foot extension ladder was not extended 3 feet above the working level. The workers were exposed to fall hazards above 11 feet to the concrete lower level.
