29 CFR 1926.950(b)(2)(ii): Each qualified employee was not trained and competent in the skills and techniques necessary to determine the nominal voltage of exposed live parts. a) Cheatham Annex Building 538: On or about 11/10/2015, a subcontractor employee was not properly trained to determine the nominal voltage of exposed live parts in the rear main circuit breaker compartment of the 15kV Distribution Switchgear, as a result an employee used a Fluke 179 True RMS Multimeter rated for 1000 volts to frisk a 4,160 volts system and was severely burned. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

In assigning an employee or a group of employees to perform a job, the employer did not provide the employee in charge of the job with all available information that relates to the determination of existing characteristics and conditions required by � 1926.950(d). a) Cheatham Annex Building 538, 15kV Distribution Switchgear: On or about 11/10/2015, subcontractor employees were not provided with the drawing that showed the location of the current and potential transformers, as a result the rear cover to the main circuit breaker compartment was removed exposing employees to an electrical shock hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
