29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) The employer failed to develop, document and utilize specific procedures to control potentially hazardous energy of the baling machines such as electrical hydraulic and kinetic energies. Employees were exposed to crushing injuries and electrical hazards, such as shock and/or burns, when performing maintenance or removing jams from the baling machines.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a)Northern end of the West side of the larger blue baler, in front of the step; Employees were exposed to electrical shock and burn hazards. Hazards were created by the lack of a junction box cover over the metal junction box containing live electrical parts.
