29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures)was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Bucyrus Precision Tech, Inc. - Bucyrus, Ohio: On or about December 4, 2015, an employee sustained amputations to the right little finger and left index finger when changing SMW bearings on Lathe 4-3 in the Counter Shaft area. The employer failed to ensure that the machine was de-energized, isolated, and that a lockout and/or tagout device was applied prior to work occurring on the machine.
