29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    (a) Bag Converting Department, Line 10 - The employer did not ensure machine operators were adequately trained as "Authorized" employees and/or knowledgeable in the recognition of minor servicing procedures that require machine shutdown and lockout to be performed to prevent injury from operating equipment.  On or about December 2, 2015, a worker was injured when his hand was pulled into an in-running nip point while clearing a jam of scrap material.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) Bag Converting Department, Line 10 - Employees who perform minor service and maintenance such as clearing jams on and around HCI Bag converting machines were not required to:      (1) Know the type and magnitude of the energy, the hazards of the energy to be controlled and the methods or means to control the energy,     (2) turn off and shut down the machine,    (3) physically locate and isolate the energy isolating devices from the energy source(s),    (4) affix lockout or tagout devices to each energy isolating device,    (5) verify that isolation and deenergization of the machine had been accomplished.    On or about December 2, 2015, a worker received a severe injury to his hand/finger while clearing a jam of scrap material from a HCI Bag Converting Machine. The worker was not adequately protected by a guard to prevent entry of any body part in to the danger area.

29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.  (a) The employer failed to record injuries to temporary workers within 7 calendar days of the incident. Temporary employee Delfino Barrasas, sustained an injury on or about August 27, 2015 that resulted in eight (8) days of "On the Job Transfer or Restriction" was not included on the OSHA From 300 as of December 9, 2015.
