A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  a. At the site, employees were exposed to a crushed by hazard, in that,  no ladder was provided as a means of access  or egress in in a trench or excavation 7 feet 8 inches deep and 14 feet wide, on or about 12/09/2015.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):         a. At the site employees were exposed to a crushed by hazard, in that, no protective system was used while laying pipe within an unprotected (trench or excavation) that was 7 feet 8 inches deep, 13 feet wide and sloped at a 45 degrees in type C soil, on or about 12/09/2015.
