Byrne Rd. Toledo, OH: The employer failed to ensure an employee working from an articulating truck mounted lift was adequately protected from falls. The exposed employee tasked with the installation of the sign was not attached (clipped-off) to the basket of the lift and was exposed to an approximate 12 ft. fall to the ground below.

Employees using tool in the unguarded condition to cut structural steel were exposed to potential contact of the blade during cutting operations.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap: (a) Cute Signs, Inc. on or about December 09, 2015, at the address of 1223 N. Byrne Rd. Toledo, OH: The employer failed to ensure the flexible cord energizing a Metabo Angle Grinder was of continuous length and insulated appropriately. The tools flexible cord maintained a splice which was wrapped with electrical tape. An employee utilizing the tool in this energized 120 volt condition was exposed to electrical shock.
