29 CFR 1910.303(b)(2): Listed or labeled electrical equipment (temporary extension cord sets) were not used in accordance with instructions included in the listing or labeling: a) On or about 12-09-15,and times prior thereto, at location: Building 146, Hampton VA Medical Center; Hampton, VA 23667: Employee(s) were exposed to electrical hazards by using four (4)100 foot temporary electrical cordsets plugged into one another, coming from an outside receptacle to power a heat gun used to glue roof targets. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees. (a) Date of Violation: On or about 12-09-2015 and times prior thereto located on top of Building 146 Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA 23667, employees conducting roofing work "punch list" items of "gluing targets" were exposed to falls from heights approximately 15 feet, 4 inches above the ground level below without any type of fall protection system, fall restraint system, safety net, guardrails, warning system, safety monitoring system, or any combination thereof. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
