29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected emerging, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 12/08/2015, a written Lockout/Tag out program had not been developed/implemented for employees engaged in maintenance operations to service such equipment as, Iron Presses, Dryers, Washers, Lint Scrubber Energenics, and Folders.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage: On or about 12/08/2015, in the boiler room area, sufficient access to electrical breaker box used for lights, boilers, air compressors, and equipment was not provided in that the area in front of it was blocked by two file cabinets.
