29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:(a)    For the facility at 306 Sarasota Center Blvd., Sarasota, FL 34240 on or about 12/03/2015, employees entered an industrial dryer to clean rollers with methyl ethyl ketone solvent (MEK). The dryer was a permit required confined space (PRCS) and a PRCS program was not established. Two hazards were present inside the confined space:    a) The dryer functioned to cure an adhesive coating that was flammable. Both the adhesive coating and MEK are flammable liquids with flash points below 20 degrees F. As a result, there was the potential for a hazardous flammable atmosphere to exist inside the dryer.     b) There was also a physical hazard present inside the dryer. An explosion panel was located on the underside of the dryer were employees worked that was activated by air pressure. In event of stepping on the panel, an employee would fall through the panel 12.5 feet to the floor below. On 12/03/2015, an employee fell through the panel and was hospitalized.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) For the establishment at 306 Sarasota Center Blvd., Sarasota, FL 34240 on or about 12/03/2015, machinery such as but not limited to, an industrial dryer. The dryer was serviced weekly to clean off rollers inside the dryer. Hazardous energy existed in the form of high temperatures (300 degrees F) and gravitational energy from an explosion panel that could open located on the underside of the dryer.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    a) For the establishment at 306 Sarasota Center Blvd., Sarasota, FL 34240 on or about 12/03/2015, machinery such as an industrial dryer was serviced weekly to clean off rollers inside the dryer. Hazardous energy existed in the form of high temperatures (300 degrees F) and gravitational energy from an explosion panel that could open located on the underside of the dryer.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:    a) For the establishment at 306 Sarasota Center Blvd., Sarasota, FL 34240, on or about 12/17/2015, confined spaces were present inside an industrial drier that contained flammable solvent vapors from an adhesive and methyl ethyl ketone used as a cleaner. This resulted in a potential flammable atmosphere inside the dryer. The confined space also contained a physical hazard, in that, employees inside could fall through an explosion panel to the floor below. The confined space was not labeled as a permit required confined space.
