Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee in that the employee hands/fingers were exposed to being caught-between and/or crushed by a 10' press brake hydraulic bender machine while performing work and/or walking in close proximity to the equipment:    a.  6443 Warren Drive, Norcross, GA (10' press brake hydraulic bender machine) - An employee was exposed to caught-between and/or crushed hazard.  The foot control 10' bar, which controls the press brake hydraulic bender machine, has an adjustable pedal which was not guarded to prevent operator and/or employees in close proximity  from accidentally stepping onto the control bar/pedal or from falling and/or moving objects to prevent unintended operation of the bender machine, which can result in serious injuries, on or about 12/09/15.      Among other methods, feasible and acceptable abatement methods to correct this hazard are:    a.   Providing a protective cover over the treadle bar to prevent unintended machine actuation as stipulated in ANSI B11.3-2012, section 6.4.4.      b.   Consult the manufacture for the replacement of the 10' foot control treadle bar, with a safety anti accidental actuation single foot control pedal for the 1984 Roto-Die Press Brake Hydraulic Bender Machine.     c. Provide a locking mechanism to lock the treadle bar in a fixed position. This locking mechanism should be designed to prevent unintended machine actuation. The locking mechanism should not allow locking in the operating position.
