29 CFR 1926.760(a)(1):  Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  a. On December 10, 2015, on the 2:12 pitch metal roof, workers installing metal decking, gutters, and trim were not protected from a 17 to 25 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:  a. On December 10, 2015, on the south side of the building, the ladder used to access the metal deck roof did not extend at least 3 feet above the eave, exposing employees to a 17 feet fall hazard.
