29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about 12/9/15 located at 7025 Parrish Way, Cumming, GA: Employees were roofing a new house with roofing shingles and shingles and tools were observed sliding off the roof from overhead. As employees were working be low on the ground and a near miss of the items hitting employees that were not provided hard hats. Employees were without hard hats exposing them to a struck by hazards.

On or about 12/9/15 located at 7025 Parrish Way, Cumming, GA:  Employees were observed roofing a new house while using a pneumatic nail gun to install the roofing shingles without safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 12/9/15 located at 7025 Parrish Way, Cumming, GA: Employees were observed roofing and installing shingles to the roof of the new house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 30 feet and a 8/12 pitch roof.
